This form must be filled out legibly and sent with a copy of a valid driver’s license and valid registration for each vehicle the permit is for.

Both driver’s license and registration addresses must match and show current legal address within the town of Porter.

This form and copies MUST be mailed to 303 Franklin St., Porter IN 46304.

Check and Money Order ONLY must accompany the request. NO CASH.

Town of Porter Residents (limit 2 per household) $45.00 
A valid driver’s license and vehicle registration must show matching current legal address within the Town of Porter for Porter Resident rate.

Town of Porter Seniors (60+ years at time of purchase - limit 2 per household) $20.00
A valid driver’s license and vehicle registration must show matching current legal address within the Town of Porter for Porter Senior rate.

Indiana Residents (limit 2 per household) $75.00
